Abstract

Introduction. Collagen sprue is a rare clinical disease that is characterized as a diarrheal syndrome that progresses to malabsorptive symptoms, weight loss and, less frequently, nausea, vomiting and abdominal pain. Its diagnosis depends on endoscopic and histopathological studies, with the main finding being an increase in the subepithelial collagen layer. Goal. The aim of this study is to present a rare and atypical case of collagenous sprue. Case description. 31-years-old female patient admitted in 2022 November with severe abdominal pain associated with vomiting after eating, weight loss and constipation. During additional investigation, endoscopy revealed a reduction in the mucosal folds of the 2nd duodenal portion and, in the anatomopathological study, findings compatible with collagen duodenitis. Conclusion. Despite a rare diagnosis, collagen sprue must be recognized so that early treatment can reverse the changes caused by this disease.
